2025 IRMAA Surcharges — Part B Premium by Income

If your Modified Adjusted Gross Income (MAGI) from 2 years ago exceeded certain thresholds, you pay an Income-Related Monthly Adjustment Amount (IRMAA) added to your Part B premium. The 2025 IRMAA is based on your 2023 tax return.

Medicare Enrollment — When to Sign Up & Avoiding Late Penalties

Joining Medicare at the right time is critical. Late enrollment can result in permanent premium penalties:

Part B Late Penalty

10% premium increase for every 12-month period you were eligible but did not enroll. This penalty is permanent and continues for life.

Part D Late Penalty

1% of the national base beneficiary premium ($36.78 in 2025) × months without coverage. Also permanent.

Initial Enrollment Period (IEP)

7-month window: 3 months before your 65th birthday month, your birthday month, and 3 months after. Enroll online at SSA.gov/medicare.

The standard 2025 Medicare Part B monthly premium is $185.00, up $10.30 from 2024. Higher-income beneficiaries pay IRMAA surcharges on top of this, up to $628.90 per month for individuals earning above $500,000 annually.
